There was an unusual possibility the other day, a package was updated a few days ago which required the initramfs to be rebuilt, but then the initramfs program was updated a few days later. If the two updates were done at the same time then ..... As part of the update apt stopped and asked whether it should use the default initramfs build or the current initramfs because it noted they were different, the default was N which iirc was the incorrect answer.
