Try flashing an official Raspberry Bookworm OS and report back.
Do you have another Pi? Use that one instead of Windows.

Failed writing at 2% .
I have only 1x IO board and 3x CM4 . All 3 have these same errors when starting rpiboot.exe

@cleverca22 what arguments should I try to run rpiboot with ?

UPDATE:
I flashed Raspberry PI OS 64bit on the uSD card (usb adapter).
Removed J2 eMMC jumper , then inserted USB ( uSD) card in IO Board.
I power IO board+ with HDMI and it does start the OS.

Does that mean the CM4 eMMC is healthy ?
